Gastroesophageal Reflux Or Heartburn

 

GERD stands for gastroesophageal reflux disease, a condition in which stomach contents (including the acids it contains)  back flows up into the esophagus (the tube through which food passes from the mouth to the stomach just behind the sternum).

The stomach is lined with a mucous that protects it against the effects of acid. Because the esophagus lacks such a protective coating, the contents of the stomach acid that flows backward (reflux) causes heartburn (also collectively referred to as dyspepsia).

Many people experience occasional heartburn.   However, those who suffer from GERD, constantly have heartburn or other symptoms of GERD.

The GERD condition affects  about 10 to 20 percent of the population of the Western world. Adults, children and even infants may have GERD.

 

Causes Of GERD

GERD occurs when the valve at the bottom of the esophagus (called the lower esophageal sphincter, or LES) that prevents the back of the stomach contents, is not working properly. Acids flow back from the stomach into the esophagus and damage its walls generate sensations of burning and pain.

 

Risk Factors For GERD Include:

Age (for people aged over 50 years)
Overweight or obesity
Ingestion of certain foods or beverages (see below)
Excess abdominal fat
Pregnancy
Smoking
Asthma
Taking certain medications (eg. NSAIDs, antidepressants, calcium antagonists, corticosteroids).

coffee, tea, cola
Foods high in acid (eg. tomatoes, orange juice)
fried or fatty foods
garlic
onions
peppermint
spicy foods.


Some researchers believe that GERD is associated with a condition called hiatal hernia in which is in the upper part of the stomach back up into the chest cavity through the diaphragm. The people with severe GERD have a hiatal hernia almost always.

 

GERD Symptoms and Complications

The main symptom of GERD is heartburn. This is a localized painful burning sensation behind the breastbone but can move up to the back of the throat. The pain can last for 2 hour and it worsens when the person eats, stretches or bends.

Because heartburn and heart attack have similar symptoms, it is important to recognize what sets them apart. In the case of a heart attack, pain burning pain travels progressively towards the end of the left arm or both arms. In the case of GERD, the burning seat in the upper chest. Gastroesophageal – Acid Reflux

The term gastro esophageal reflux in the esophagus indicates the rise of what is contained in the gastric cavity (stomach), this eventuality is very frequent and almost daily in most subjects even in the absence of pain.

The gastro esophageal reflux is mainly caused by the conjunction of several causes:  a prolonged stagnation of food in the stomach which empties too slowly.

The failure or incomplete functioning of the cardia causes the gastric contents, acid sometimes associated with pancreatic juice and bile reflux into the esophagus.

The longer the acid content of the stomach remains in contact with the mucus lining of the esophagus ( several hours during sleep at night), the more severe the injuries that this contact will produce in the mucosa of the esophagus. Giving rise to different grades of esophagitis (inflammation of the lining of the esophagus).

The gastro-esophageal reflux disease (GERD) occurs when the gastro esophageal reflux is more prolonged and more episodes are not infrequent, but very often throughout the course of 24 hours, especially after a big meal in overweight people

These acid reflux symptoms are added to the burning reportedly behind the breastbone which can also radiate to the back between the shoulder blades.  Painful swallowing (odynophagia) caused by the passage of food on the inflamed esophageal mucosa, dysphagia (difficulty in passage of food along the route of ‘esophagus as a result of narrowing of the lumen due to ulcer healing after a severe esophagitis).

Pain in the cardia (the entrance of the stomach) caused by the presence of lesions including ulcerative, the last stretch of the mucosa’ esophagus which can lead to cellular changes in the esophageal called “Barrett’s esophagus.”

Milk has a beneficial effect, but especially if you overdo it with the amount -  after the initial relief,  it can cause a rapid return of heartburn symptoms.

 

Further Acid Reflux Diet Advice

 

Drink more fluids to protect the esophagus from stomach acid mossy.

Stop smoking:  Smoking promotes acid reflux, increases gastric acidity and makes it more susceptible to acid attacks to the lining of the stomach.

Do not abuse of some drugs:  Such as NSAIDs (aspirin, ibuprofen, some sedatives and tranquilizers, etc.).. It is recommended in all cases to notify your doctor of use, so he can check for compatibility with the disease and find possibly more healthy alternatives, than acid reflux medications.

And stress?

Gastroesophageal Reflux: What Is It?

The gastroesophageal reflux means part of stomach contents had leaked into the esophagus (the tube connecting the mouth to the stomach).

The stomach produces strong acid (gastric juice) that help digest food.

However, the lining of the esophagus is not designed to withstand the acidity of stomach contents.. This causes  esophageal inflammation, which results in burning and irritation. Over time it can cause damage to the esophagus.

 

The Causes Of Heartburn


For most people, the acid reflux comes from a malfunctioning lower esophageal sphincter. This sphincter is a muscular ring at the junction of the esophagus and stomach. Under normal circumstances, it prevents the stomach contents from backing up into the esophagus…Opening only to let the eaten  food eaten into the stomach. (thus playing a protective role) .

In the case of acid reflux, the sphincter opens and lets the gastric juices of the stomach flow into theesophagus . Sufferers often have  acid reflux after a meal or overnight.  This phenomenon of regurgitation is very common in infants, because the sphincter is immature.

Gastroesophageal reflux may also be related to a hiatal hernia. In this case, the part of the stomach at the junction of the esophagus “back” into the chest cavity through the orifice of the diaphragm, which normally allows  the esophagus (the hiatus).  However, these two conditions are not synonymous, and hiatal hernia is not always associated with acid reflux disease.

Prevalence

In the United States, 44% of the adult population have reflux at least once a month. . In Canada, an estimated 29% of the population would be inconvenienced by occasional episodes of gastroesophageal reflux.

Regurgitation is extremely common in infants.  Acid reflux in infants is not necessarily attributable to gastroesophageal reflux. Experts estimate that 25% of infants have a true acid reflux

Evolution

In the majority of adult patients, acid reflux symptoms are chronic and appear in recurrences. . Acid reflux treatments offer the most complete relief, but temporary, symptoms.. They do not cure the disease.

In infants, reflux usually resolves as the child grows – between 6 and 12 months

 

Complications


Prolonged exposure of the esophagus to acidic gastric substances can cause:

* severe inflammation and lesions of the esophagus (esophagitis), especially among the elderly;
* ulcers (or ulcers) on the wall of the esophagus;
* a narrowing of the diameter of the esophagus (peptic stricture), resulting in difficulty swallowing and pain during swallowing;
* This is the replacement of cells in the lining of the esophagus by cells that normally move in the intestine.This replacement was due to “attacks” repeated gastric acid into the esophagus. It is accompanied by any symptoms, but can be detected by endoscopy because the normal gray-pink tissue of the esophagus is inflamed pink salmon color.  . Barrett’s esophagus puts them at risk of ulcers and, more importantly, cancer of the esophagus .

Note that people with acid reflux disease are at risk of dental problems because of the increased acidity in the mouth. It also seems that the gastroesophageal reflux may exacerbate or trigger asthma.

 

When to Consult a Doctor?


If you experience one of the following situations, it is advisable to consult a doctor.

* A burning sensation and acid regurgitation several times a week.
* Reflux symptoms disturb sleep.
* The symptoms recur quickly when you stop taking antacid medications.
* The symptoms last for over a year and have never been evaluated by a doctor.
* There are alarming signs (see below).


Symptoms of gastroesophageal reflux

 

The main symptoms of acid reflux, especially after meals or when lying down:

* Burning “up” behind the breastbone. The doctors talk about heartburn.
* Acid regurgitation, giving a bitter taste in the mouth.

Reflux can also lead to symptoms less frequent and more general:

* Hoarseness, especially in the morning
* A sore throat chronic
* Of asthma occurring at night and unrelated to an allergy.
* Chronic cough or frequent hiccups
* Nausea
* Persistent bad breath
* Dental problems (loss of tooth enamel)


Symptoms Of Infants Acid Reflux Are:

*  excessive regurgitation and (or) vomiting;
*  pain, refusal to drink, fits of crying;
* growth retardation and anemia in severe cases.


Alarming Symptoms

Alarming symptoms that should lead you to consult a doctor immediately, because they may indicate a complication or another disease.

* Difficulty swallowing.
* Pain during swallowing
* Coughing, wheezing and a repeated need to clear throat.
* Stomach aches and vomiting
* Weight loss
* Sputum (phlegm), which contains blood, or blood in vomit or stool (black stools).


Good to know – The pain caused by the burning of reflux can sometimes be intense to the point of thinking of a heart attack.  Consult a doctor if you experience any unusual pain in the chest that radiates to the arm or jaw, as it can really be a heart attack.

 

People At Risk


* People who have a hiatal hernia (see above).
* Pregnant women, during the last months of pregnancy.  The fetus exerting additional pressure on the stomach, reflux in this case is only temporary.
* Obese or overweight
* People aged 50 and older. With age, some individuals have less efficient esophageal sphincter, which can cause gastroesophageal reflux.
* People with scleroderma .


Risk Factors

Smoking (cigarettes, cigars, pipe) increases the risk of gastroesophageal reflux.  Smoking cessation, with the adoption of other lifestyle measures can help alleviate acid reflux disease symptoms.

 

How to Prevent Gastroesophageal Reflux?

Smoking and obesity contribute significantly to the onset of acid reflux. Not smoking and maintaining a healthy weight are therefore are a great means of prevention. But in the current state of knowledge, in most cases it does not appear to be other ways to prevent the onset of acid reflux disease.

 

Measures To Reduce Symptoms And Prevent Recurrence

Try to find out what lifestyle changes can help reduce your symptoms and discuss with your doctor or nutritionist. These habits can vary from one person to another.

However, for most people with acid reflux, it is easy to adopt the right measures to:

* If you are overweight on the scales, try to lose a few pounds.  Being overweight and obesity are major risk factors for symptoms of acid reflux.  Several studies have shown that voluntary weight loss is effective in reducing symptoms in a sustainable manner.
* No studies showed a clear link between foods and the aggravation of reflux… However, people with acid reflux often observe that certain foods aggravate their symptoms, including coffee, chocolate, fatty foods, spicy foods, carbonated beverages, citrus fruits (and their juice), tomatoes, milk and onions. Avoid eating these foods may help reduce acid reflux.
* Avoid drinking peppermint and spearmint, which could affect the relaxation of the sphincter of the esophagus.
* Avoid fatty meals.  Some studies have noted that meals high in fat aggravated reflux, although this hypothesis remains controversial.  In all cases, prefer rather light meals but more frequently to facilitate the “emptying” of the stomach and prevent acid reflux.
* Eat slowly, taking time to chew food avoids overloading the stomach.
* Avoid lying down after eating (wait two to three hours).
* Avoid eating for three or four hours before bedtime.
* For smokers quit …! Smoking slows healing of esophageal lesions and affects the operation of his sphincter.
* Reduce your consumption of alcohol. Alcohol increases acid secretion in the stomach and aggravates the reflux. Especially, do not drink alcohol on an empty stomach.
* Avoid tension and stress.
* Avoid tight-fitting clothing and belts as they can exert pressure on the abdomen which can be quite strong for the lower esophageal sphincter opens, then it should remain closed.
* Elevate head of bed at least 15 centimeters (6 inches). If possible, put wooden blocks under the legs of the headboard or use a solid piece of foam under the portion of the mattress where the head rests (sleeping with a few extra pillows is not enough). This measure is effective in reducing night pain, but can be uncomfortable and interfere with sleep in some people.
* Some drugs can cause symptoms of acid reflux or contribute to irritate the esophagus: acetylsalicylic acid (aspirin) and other anti-inflammatory drugs (ibuprofen), drugs against osteoporosis (Fosamax ®, calcium) antibiotics, some sleeping pills … supplements (iron, potassium) and plants can also aggravate symptoms.  Consult your doctor or pharmacist.

 

Medical Treatment of Acid Reflux

Drugs

Before taking a drug against GERD, it is essential to put in place measures to prevent recurrences listed above and change some habits. If this is not enough, people who regularly experience acid reflux pain can get a simple and effective treatment to relieve their symptoms.  However, the reflux is a chronic disease, it is sometimes necessary to continue long-term acid reflux medication.

Several medications can be administered to decrease gastric acidity:

* antacids (Maalox ®, Rolaids ®, Tums ®), which neutralize stomach acid, to take in case of symptoms. If you must use the-counter antacid for more than three weeks, it is necessary to consult your doctor;
*  H2 antagonists (Axid ®, Pepcid ®, Zantac ®, Tagamet ®), which decrease the production of stomach acid. H2 antagonists are available OTC and are usually sufficient to treat mild cases, together with prevention tips outlined above. That said, it is not advisable to take antacids for long periods as they may interfere with the absorption of some nutrients;
* inhibitors of proton pump inhibitors (PPIs). If antacids or H2 antagonists obtained over the counter does not completely relieve the symptoms, the doctor then prescribed stronger H2 antagonists (or use more frequently) or PPI (Losec ®, Nexium ®, Pantoloc ®, Pariet ®, Prevacid ®). These are the most effective medications for treating reflux, but they can cause side effects if not taken properly.. It is therefore essential to follow the advice of your physician.

Moreover, in addition to medication, the doctor sometimes recommended to exclude or limit certain foods.  See the Prevention section.

Good to know -  Multivitamins are sometimes recommended by some people who are specialists in anti-reflux medication to avoid some deficiencies.  Indeed, anti-reflux may decrease the absorption of nutrients and certain drugs. If you follow another treatment, consult your doctor or pharmacist.

 

Surgical Treatments

In case of failure of drug therapy, surgery to treat gastroesophageal reflux disease is possible, but it is mostly reserved for patients with severe complications, and it is rarely necessary. However, surgery is more common in cases of hiatal hernia.I Cured My Acid Relfux

Acid reflux and GERD disease is the abnormal “backups” of fluid in the stomach into the esophagus.
